Conditions treated within 24 months prior to enrollment phase in over the first few membership years.
Conception must occur after six months of continuous Plus membership for maternity sharing to apply.
Households with one or more tobacco users pay an additional $105 per month.
A household won’t be responsible for more than three IUAs in a rolling 12-month period.
